1. Scan of S1 diploma and S1 transcript (combined in 1 pdf file)
2. ID card scan
3. Scan of Family Card (KK)
4. Scan of NPWP
5. 4×6 photo file (photo provisions please see the initial link of self-report of lambung mangkurat university)
6. Scan of Criminal Record Certificate (SKCK) from police station, police station or state police. Please adjust accordingly.
7. Scan of drug-free certificate (SKBN) from a hospital or health facility clinic
8. Scan of Doctor’s Certificate (physical) from hospital, health center or health facility
9. Scan of the Intergrity Pact that has been filled in, signed and affixed with a 10,000 stamp duty.
